Exceed Team helps independent support workers, provider businesses and approved family members work together around the people they support. A SHARED PLACE FOR EACH CLIENT Keep preferences, communication needs, routines, contacts and NDIS goals together. Personalise the client overview and choose the questions that matter for their support. SHIFT NOTES THAT FIT THE PERSON Write notes from the shift, link them to goals and capture the participant’s own words. Use custom questions and optional sections. Choose Client’s team, My provider or Just me before submitting. Corrections are added as amendments so the original note remains available. ROSTERS AND HANDOVERS See upcoming shifts, accept or decline assignments, take available shifts and record start and finish times. Check recent notes and follow-ups as you prepare for the next shift. WORK ACROSS PROVIDERS Each person has their own account. Client access is managed through an approved support team, so independent providers can collaborate without sharing logins. Post team updates, reply and keep important information together. INCIDENT RECORDS Record incidents and reporting dates, with reminders to help the responsible provider follow up. Exceed Team does not submit reports to the NDIS Commission or replace professional judgement or a provider’s reporting obligations. PRIVACY AND SUPPORT Database access is restricted by client team and note audience. The database is hosted in Sydney, Australia. No advertising or advertising tracking.
