SPHERE HEALTH: Virtual Care

The SPHERE app is not a medical device. SPHERE HEALTH is a technology platform and does not provide medical care. All clinical care is provided by independent, licensed physicians. Expert care for every season of a woman's life. SPHERE HEALTH is a telehealth app for women's hormonal and metabolic health. You complete a structured health assessment, your labs are ordered and reviewed by a licensed physician, and you get written results and a care plan you can act on, with your care team a message away. WHAT YOU CAN DO IN THE APP • Complete the SPHERE HEALTH Assessment, a structured intake about your symptoms, history and goals • Get a condition-curated lab panel ordered for you, drawn at a partner lab or, on the Constant plan, at home • Read your results explained in plain language, alongside the ranges that matter for your condition • Read the written care plan your physician creates from your labs and your intake • Message your care team between visits, with clinical review of your questions • Keep your documents, prescriptions and care history in one place • Track how you are doing over time with check-ins WHO IT IS FOR SPHERE HEALTH supports four condition pathways: • PMOS (formerly PCOS) • Endometriosis • Perimenopause and menopause • Metabolic health PLANS Clarity, one-time. A complete hormonal picture, reviewed by a physician. One payment, no membership. Includes a full review by a board-certified OB-GYN who specializes in your condition, written results and a care plan explaining what your labs mean and what to do next, and a clear recommendation on whether ongoing care is right for you. Clarity does not include prescriptions or ongoing visits. Continuum, membership. Physician care and continuous management of your condition. Includes OB-GYN visits, a full lab panel at the start and re-baselining once a year, prescription management when your physician judges it clinically appropriate, and messaging with your care team between visits. Constant, membership. Concierge care for when convenience matters most. Everything in Continuum, plus at-home blood draws, lab re-baselining every six months, priority response from your care team, free shipping on medications, and unlimited messaging. Medications, when prescribed, are billed separately at the pharmacy's actual cost, itemized, and you review every charge before it is applied. HOW CARE WORKS Visits are asynchronous: you and your physician exchange secure messages rather than meeting at a set time. Your physician reviews your intake and your labs, writes your plan, and adjusts it as your results and symptoms change. WHO PROVIDES YOUR CARE SPHERE HEALTH is a technology platform, not a medical group, and does not practice medicine. Medical care is provided by an independent medical group and its licensed clinicians. The app itself does not diagnose, prescribe or dispense: it collects your information, routes it to a licensed clinician for review, and shows you what that clinician decides. Every prescribing decision is made by your physician. YOUR PRIVACY Your health information is protected health information and is handled under HIPAA. There are no advertising SDKs, no tracking pixels and no third-party analytics behind the sign-in. You can request deletion of your account and your data from inside the app, in Settings. You must be 18 or older to create an account. SPHERE HEALTH is not for medical emergencies. If you have a medical emergency, contact your local emergency services.
