CONNECTED ON YOUR TERMS Apple Health access on your iPhone is free, optional, and read-only. You choose what Form can read. Cloud health trend sync and Health insights for Coach require Form Plus and separate permissions. Coach and food-photo analysis use OpenAI with your permission. One optional AI sharing choice covers both features; you can change it in Settings. Photo results are estimates you can edit before adding a meal. Form deletes its uploaded photo after processing; provider retention is explained in the privacy policy. Form is for adults 18+ and general fitness and wellness.
