PKD Compass is a personal health journal for people living with polycystic kidney disease (PKD). Keep your recorded results, daily logs, medications, documents, and appointments together for conversations with your healthcare team. Charts, reference ranges, and status indicators are informational aids, not a diagnosis, prediction of disease progression, or confirmation that your kidney function is stable. Your clinician should interpret results in your individual clinical context. Blood pressure log Enter readings from your blood pressure monitor and organize them into a weekly log with averages and a shareable report. PKD Compass does not measure blood pressure, continuously monitor your health, or alert a clinician when you record a concerning reading. Follow your clinician's measurement schedule and instructions. Medication journal and reminders Record medications, prescribed doses, schedules, logged doses, and remaining supply. Reminders help organize your routine; they are not prescribing or dosing advice and may be missed or delayed. Verify medication instructions with your doctor or pharmacist. Food and fluid diary Log meals, estimated nutrients, and fluids. Compare recorded intake with configured targets. Estimates and default targets may not be appropriate for you; agree on individual nutrition and fluid goals with your doctor or renal dietitian. Document vault Keep copies of reports and visit documents. Optional automated document summaries can be incorrect or incomplete. Check the original document and confirm findings with your clinician before acting. Care organizer and resources Keep personal appointments, tasks, and timeline entries together, and explore kidney-health resources. Recording an appointment does not book a visit with a provider.
