About
FormKiosk turns an iPad or a spare iPhone into a feedback terminal for your counter. Someone taps a face, the screen thanks them, counts down and resets for the next customer. About four seconds, start to finish. No app for them to install, no account for them to make, nothing for your staff to reset between customers. BUILT FOR A COUNTER, NOT FOR A DESK The form fills the screen and the controls are sized for someone standing up in a hurry. Turn on Guided Access and the iPad stays on your form: a customer cannot wander off into Safari or the home screen. The screen stays awake all day and dims to a tap-to-start screen when nobody is around. IT KEEPS WORKING WHEN THE WIFI DOES NOT Answers are written to the device the moment someone taps Send, and they send themselves when the connection comes back. A dropout costs you nothing. The time recorded is when the customer actually tapped, not when the wifi returned, so your charts stay honest. NO PASSWORD ON THE COUNTER The device shows a six-character code and you claim it from your phone at formkiosk.com/pair. What the device holds is access to one form and nothing else: it cannot read your responses back, see your other forms, or reach your billing. Unpair it from Settings whenever you like, without changing anyone's password. Staff can protect the device's own panel with a PIN. IT LOOKS LIKE YOUR BUSINESS Your colours, your wording, light or dark. Smiley or star ratings, multiple choice, yes or no, free text, NPS. Build the form on the web, and the device picks up the changes on its own. YOU NEED A FORMKIOSK ACCOUNT This app runs forms you have already built at formkiosk.com. New workspaces get a 14-day free trial, no card required. The app itself is free and has no purchases inside it. formkiosk.comRead more

Forms can now be in Romanian. Set a form's language on the web and everything the app adds around your questions follows it: the Send button, Yes and No, the rating labels, the error messages, the thank-you and the restart countdown. Your own questions stay exactly as you wrote them. You can also choose your own text for the Send button.
