Support Scan Rounds

Support Scan turns safety rounds into verified, defensible records. Behavioral health and treatment facilities are required to observe clients on a schedule — every 15, 30, or 60 minutes — and to prove it. Paper sheets and clipboard initials can be filled in after the fact, and they cannot show a surveyor that anyone was actually in the room. Support Scan can. PROXIMITY VERIFICATION Staff carry their phone. Each client wears a small wristband. When a staff member records an observation, the app confirms the phone was physically near that wristband using Bluetooth, or by tapping the band directly. A check that cannot be verified is still recorded — clearly marked, with the reason the staff member gave. Nothing is silently dropped, and nothing can be back-filled from the nurses' station. BUILT FOR THE PACE OF A REAL SHIFT The rounds screen shows who is due, who is coming due, and who is overdue, sorted by urgency, in one glance. Recording an observation takes seconds: tap the client, tap what you observed, tap where. Walk mode moves through the whole unit without returning to the list. WORKS WHEN THE WIFI DOESN'T Observations recorded in a dead zone are stored on the phone and sync automatically when signal returns, with their true times preserved. SUPERVISORS SEE EVERYTHING, LIVE A live dashboard shows the census, compliance, overdue rounds, and manual overrides as they happen. Push notifications escalate a missed round immediately rather than at the end of a shift. AUDIT-READY REPORTING Run an individual client audit for the first 24, 48, or 72 hours of care — every round that was expected against what actually happened, with times, staff, and verification method. Print it or save it as a PDF for DHCS, the Joint Commission, or your own quality review. Records are append-only: entries cannot be edited or deleted after the fact, and corrections appear as new entries referencing the original. DAILY PROJECTS Assign operational tasks to staff — deep clean the refrigerators, audit the cash logs — and require two photos to mark one complete. The photos reach the administrator with the record.
