The mZUS for Płatnik mobile app provides quick access to key information and services related to ZUS contribution payer settlements. You can conveniently check your balance, receive messages from ZUS, manage documents, submit applications, and make payments – all from your mobile device. Who is the app for? The mZUS for Płatnik app is designed for all contribution payers who have an active eZUS account. It can be used by both entrepreneurs and individuals managing contribution settlements on behalf of companies, institutions, or multiple payers simultaneously. The app is useful for users who need: • quick access to balances and settlement information, • the ability to make electronic payments, • convenient receipt of messages and correspondence from ZUS, • access to e-ZLA issued for insured individuals, • the ability to send applications and documents to ZUS from a mobile device, • easy switching between profiles of different payers. What features does the app have? With the mZUS for Płatnik app, you can: • manage settlements: o check the payer's balance from individual funds and declaration number ranges, o view declarations and payments to the payer's individual accounts, o initiate electronic payments; • read notifications, correspondence, and communications from ZUS; • view e-ZLA: o check information on medical certificates for individuals registered for insurance, o display the e-ZLA list from the current day, current month, or previous month, o search for e-ZLA exemptions based on selected criteria; • submit applications and documents: o submit RD-3 and RWN applications, and A1 certificates (US-1, US-2, US-3, US-4 and appendices US-54 and US-55), o create and submit RUD documents (notification of a contract for specific work), o view submitted applications and documents, o generate certificates with payer details yourself; • Use ZUS services: o Book in-person and online appointments, o View and cancel scheduled appointments, o Call the ZUS Customer Contact Center, send a query to ZUS, or visit the eZUS and zus.pl websites; • Manage your profile: o View current profile owner identification information, o Switch between different contribution payers, o Configure the security, appearance, and settings of the app, o Log in to eZUS using a QR code. What do you need to do before installing the app? Make sure that: • You have an active eZUS account, • Your contact information (phone number or email) is up to date – you will need it for activation, • You have access to the profile of the payer whose data you want to manage. Detailed information on how to create an eZUS account can be found at www.zus.pl. How do I activate the app? To start using the mZUS for Płatnik app: 1. Log in to eZUS: https://www.zus.pl/ezus/logowanie. 2. Select Settings → Mobile Devices. 3. Click New Device. 4. A QR code (scannable) and a string of characters (manually enterable) containing the device activation code will appear on the screen. 5. Launch the app on your mobile device. 6. Read and accept the terms and conditions. 7. Scan the eZUS QR code or enter the code manually. 8. Enter the verification code you received via your phone number or email address, using the method you have set up for regaining access to eZUS. 9.
