Transform Now Health gives members one secure place to view their health records and stay connected with their care team. WITH TRANSFORM NOW HEALTH YOU CAN • View clinician-reviewed laboratory records, source reference ranges and result history • Read records supplied by connected genetic and epigenetic providers • View plans shared by your care team and record daily progress • Send secure messages and share your goals and preferences with your care team • Review appointments, supplements, orders and connected sources • Use optional food logging and still-frame exercise observations for general wellbeing PRIVACY AND CONTROL You choose which optional data sources and camera tools to enable. Account settings let you request data export, correction, consent withdrawal and account deletion. Raw camera images are discarded after analysis. ACCESS AND CARE The app is free to download and intended for adults aged 18 and over. Access to clinic records and care-team services requires an eligible account and clinic connection. Downloading the app does not enrol you in medical care. Transform Now Health is a patient portal and wellness log. It is not a medical device and does not diagnose, treat, cure or prevent any medical condition. It does not generate treatment decisions or supplement doses. Messages go to your human care team.
