willowbridge care is the mobile companion to the willowbridge platform, built for the nurses, medical assistants, and physicians who run Medicare care-management programs (chronic care management, principal care management, behavioral health integration, remote patient monitoring, transitional care, and more). FOR CARE TEAMS • Today's schedule: see who is coming in and which care-management programs each patient is a candidate for • Worklist: calls due, care-plan reviews, check-ins to triage, and RPM alerts, ordered so the next right action is on top • Patient charts: care plan, problems, goals, medications, readings, consents, and documentation in a fast, phone-first layout • Encounter timer: audit-ready time capture that follows you between chart sections and pauses when you step away • Billing: review and sign off monthly billing periods with the supporting evidence attached BUILT FOR HEALTHCARE • Sign-in protected by your practice account; fingerprint or face unlock keeps health information private on shared or lost devices • Health information travels over encrypted connections from your practice's willowbridge tenant and is not kept by the app; only your sign-in and security keys are stored, in the phone's secure storage (signed advance directives open in the phone's browser) • No ads, no tracking, no data sales willowbridge care requires a willowbridge practice account provisioned by your clinic.
