Developed in partnership with Mount Sinai Fuster Heart, the app pairs with your blood pressure monitor to make home readings simple, secure, and actionable — and keeps your care team in the loop. Key features: • Bluetooth pairing with your Omron blood pressure cuff for one-tap reading capture • Automatic categorization of each reading (Normal, Elevated, Stage 1, Stage 2) • Reading history with trends and personalized insights • Direct connection to your nurse coach for follow-up calls and support • Reminders and re-engagement prompts if your readings indicate elevated risk • Secure storage of your health information using industry-standard encryption • HIPAA-compliant data handling with audit logging The app is provided as part of a physician-supervised remote patient monitoring program for adults managing high blood pressure.
